The mission of the trans navigator is to assist trans patients to navigate through appropriate agency programs and to our outside referral network, enhancing the overall patient experience.



The Borrego Health Transgender Navigator Specialist is a patient-centered service delivery

intervention based on the goal of improving health outcomes, increasing provider-

patient connectedness, and eliminating barriers to care that can occur across the care

continuum for vulnerable trans populations.


What does a Trans Navigator Specialist do?

Following are some highlights of the Trans Navigator duties:

Administrative Functions

  • Develop and moderate regional peer support groups

  • Assist patients in accessing appropriate transgender health resources such as navigating name/gender marker changes, potential surgeons, etc.

  • Provide community-based trans training or workshops on related health and wellness topics as appropriate

  • Establish and maintain positive community relations

  • Conduct community outreach

  • In collaboration with the Trans Community Health Manager, educate staff on transgender health topics

  • Collaborate with interdisciplinary department members in assessing trans-specific needs

  • Help ensure trans referral and resource lists stay up-to-date


Clinical Functions

  • Provide information, education, and emotional support to patients in a compassionate, culturally sensitive, and appropriate manner

  • Complete brief assessment of needs with patients who may benefit from relevant services and provide referrals when necessary

  • In collaboration with Referrals team, facilitate referrals to appropriate outside providers

  • Be the contact person for patients to report positive or negative experiences with outside provider(s)
